چکیده

In the past ten to fifteen years, investigators have demonstrated conclusively that some injuries about the wrist have profound long-term consequences relative to function of the wrist. Problem fractures and non-unions of the scaphoid have been associated, in particular~ with major alterations in the kinematics of the wrist8’64. Considering its vulnerable blood supply~-4, irregular shape, peculiar oblique alignment across the plane of motion of the mid-carpal joint, and complex function as a link between the proximal and distal carpal rows2t, it is not surprising that the carpal scaphoid presents a number of diverse challenges to clinicians seeking optimum methods of treating such fractures. Despite previous reports of rates of union that exceeded 95 per cent after the wrist was treated with simple immobilization in a cast lO.t 1,~3.14,18,38,40.41,48,53, recent studies of scaphoid fractures

Comparison of short and long thumb-spica casts for non-displaced fractures of the carpal scaphoid.

A prospective study was undertaken of fifty-one patients who were randomly assigned to treatment with either a long or a short thumb-spica cast for a non-displaced fracture of the carpal scaphoid. The duration of follow-up was at least until union; the average follow-up was twelve months. Twenty-eight fractures were treated with a long thumb-spica cast and twenty-three, with a short thumb-spica...

Pulsed Electromagnetic Fields in the treatment of fresh scaphoid fractures. A multicenter, prospective, double blind, placebo controlled, randomized trial

BACKGROUND The scaphoid bone is the most commonly fractured of the carpal bones. In the Netherlands 90% of all carpal fractures is a fracture of the scaphoid bone. The scaphoid has an essential role in functionality of the wrist, acting as a pivot. Complications in healing can result in poor functional outcome. The scaphoid fracture is a troublesome fracture and failure of treatment can result ...

Percutaneous screw fixation without bone grafting of scaphoid non-union.

BACKGROUND Delays in diagnosis and inadequate treatment of acute scaphoid fractures can lead to non-unions, presenting surgeons with unique challenges regarding optimal management. OBJECTIVES To evaluate the clinical and radiographic outcome of scaphoid non-unions treated with percutaneous screw fixation. METHODS The study group comprised 12 patients with scaphoid non-unions of an average d...

Current methods of diagnosis and treatment of scaphoid fractures

Fractures of the scaphoid bone mainly occur in young adults and constitute 2-7% of all fractures. The specific blood supply in combination with the demanding functional requirements can easily lead to disturbed fracture healing. Displaced scaphoid fractures are seen on radiographs. The diagnostic strategy of suspected scaphoid fractures, however, is surrounded by controversy. Bone scintigraphy,...

A new variant of scaphoid reconstruction: Treatment of scaphoid non-union with avascular bone interponate and high compression screw (Synthes®)

Scaphoid fractures as frequently overseen injuries often result in scaphoid non-unions, that need to be treated to prevent carpal collapse and secondary cartilage damage. Vital bone tissue and compression of fracture and bone graft ends seem to be crucial in for ossification and final bone healing. In the present study we compare our results using a high compression screw (HCS Synthes(®)) to re...
